In most cases, it is faster to get medals if you work through a congressional office. However, due to the privacy act of 1974, I need your signed written permission to obtain medals on your behalf. If you are owed medals from your service, just fill out this Authorization Form. Be sure to included your name, the veteran's name (if requesting medal's for a family member) rank, social security number, service number if applicable, unit or units served in while in the military, return address and day time phone number. Fax the completed form to my district office at 614-818-0887, or mail it to my District office at:

Be sure to include documents which show the medals you were awarded, such as your DD214, or the award certificates.
